This paper is focused on studying the practices of interventions which are administered to bring holistic change on adolescents with disabilities (ages 12- 18) in two organizations, why and how they are administered and the process of interventions to understand and come up with suggestions of future intervention plans. A qualitative research method is used as a research method in this study. Data was collected using data collection methods such as key informant interviews, semi structured interviews, focus group discussions and document reviews. The interview participants were beneficiaries who were under the process of the interventions and those who have finished the intervention. The organizations employees who assume leadership roles have been interviewed to get the necessary data of the administration of interventions. The researcher also analyzed story documents of that have been compiled by the organizations. The result of the study shows that the intervention types which are medical, psychosocial and rehabilitation interventions are given according to the standard and expectations of organizational goals, but the challenges faced by adolescents with disabilities in the community remains. It is the goal of this research to suggest intervention practices which improve the life of adolescents with disabilities
