Introduction: Improving health institution discharge processes, decreasing readmissions andrnemergency department visits after discharge is on the countrywide issue for health carernreform and health facility-based improvement tasks, which are unplanned, adverse,rnpotentially avoidable, and costly outcomes of hospitalization caused by low patient readinessrnfor hospital discharge. A patient's readiness for Hospital Discharge is defined as a patient‟srnassessment of their own preparedness for discharge and their ability to cope with their illnessrnat home.
