Background: In low and middle-income countries including Ethiopia, small for gestationalrnage is a serious health problem, due to an increase in fetal and neonatal mortality andrnmorbidity. In Ethiopia and particularly in Tigray there is a paucity of information onrnincidence rate and predictors of small for gestational age among pregnant women.rnTherefore the aim of this study is to determine the effect of preeclampsia on the incidence ofrnsmall for gestational age among pregnant women in Mekelle General Hospital and AyderrnComprehensive Specialized Hospital, Tigray, Northern Ethiopia, 2019.rnMethods: A retrospective cohort study was employed to compare the small for gestationalrnage between the exposed; preeclampsia (n=239) and non exposed; normotensive (n=476)rnwomen who were antenatal care follow-up in Mekelle General Hospital and AyderrnComprehensive Specialized Hospital from January 01, 2014 to March 31,2019. Systematicrnsampling was used to select the study participants. A pre-tested structured data abstractionrnchecklist was used to extract data. Collected data were entered and cleaned using Epi Datarnversion 3.1 and exported to STATA version 14 for analysis. The incidence rate wasrncalculated dividing all small for gestational age cases in the cohort for the person-weeks ofrnfollow-up. The data analysis was performed by Cox proportional hazard model.rnResults: The incidence rate of small for gestational age was higher among preeclampsia thanrnnormotensive women (94.5 versus 24.9 per 1000 person weeks, Z=9.42, p
